Toledo, Ohio-based ProMedica health system is on the social determinants of health (SDOHs) in hopes of having a “pivotal impact” on its patients’ health. ProMedica surveyed patients in all its care settings to identify social needs and then intervened with food clinics, financial coaching and social programming. The health system also teamed up with a philanthropist, a data analytics firm, community development partners and Bitwise Industries, allowing ProMedica to address housing insecurity and job training as well.
