Case study: Team-based care leads to improved value

An inner-city primary care clinic serving a large refugee population is the subject of the latest from ÍÃ×ÓÏÈÉú’s Value Initiative. The decision by CHI Health and Creighton University Medical Center in Omaha, Neb., to make the clinic a learning laboratory for interprofessional team-based care improved value for patients, many of whom endured chronic pain and mental illness. Patients who received care in the team-based model had a 16.7% reduction in emergency department visits and a 17.7% reduction in hospitalizations. Patients also had a 48.2% reduction in charges and improved patient satisfaction. Additionally, staff engagement and satisfaction soared following adoption of the team-based care model.
