Webinar shows age-friendly care aided by community partnerships

An Age-Friendly Health Systems on June 15 at 12:00 p.m. CT will describe a framework to address older adults’ unique needs, as well as how to integrate age-friendly care in the community. Monica Long, MSN-Ed, RN, a nurse educator from the SHARE Network, will discuss how her team is implementing age-friendly care successfully through community partnerships. Participants also will learn how to participate in ÍÃ×ÓÏÈÉú’s Age-Friendly Health Systems Action Community convening this fall. Participants must register in advance for the webinar. Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with ÍÃ×ÓÏÈÉú and the Catholic Health Association of the United States.
